DESCRIPTION:Community palliative care provides support and specialised medical care to manage pain\, symptoms and emotional distress outside of hospital. \nPrimary healthcare providers play a key role as the main point of contact\, delivering consistent care and personalised assistance. \nGPs\, GP registrars and nurses are invited to meet the local palliative care team and specialist who will provide information about services available as well as outlining referral processes. \nDinner will be provided and there will be plenty of opportunity to network with peers\, palliative care team members and Murray PHN staff. \nClick here to register.
