DESCRIPTION:Hosted by Menzies School of Health Research \nJoin presenters Professor Brandy Wicklow\, a Paediatric Endocrinologist and Clinician Researcher from The Children’s Hospital Research Institute of Manitoba and Dr Joanna McLintock\, a Consultant Clinical Psychologist at Te Whatu Ora\, New Zealand with over 20 years’ experience in diabetes and mental health care. \nThis webinar brings together international perspectives on how wellbeing and mental health can be recognised\, prioritised and supported as an integral part of youth diabetes care. Presenters will share their experiences\, approaches and practical strategies for embedding psychosocial care into everyday diabetes management\, offering insights into what works across different healthcare settings. DESCRIPTION:Palliative Care Victoria received funding from the Commonwealth and State government to develop a culturally safe trauma-informed palliative care plan in partnership with Aboriginal and Torres Strait Islanders people to support Elders living in Residential Aged Care Home (RACHs). \nIn collaboration with our community partners\, we have developed the ‘Yimgawugel bungim yiya’ -Songlines for Deep Dreaming: Let’s Yarn Cultural Considerations for Palliative Care Plan that supports Aboriginal and Torres Strait Islanders patients and their families to document their cultural care considerations while they access culturally safe and effective palliative and end of life care. \nThe implementation of this document will expand on existing models of care while supporting new approaches to the way care is delivered or commissioned to our First Nations Elders within Victoria. \nThis session will introduce you to this document and provide an opportunity in how your practice can implement the plan and be part of the current project trial. \nTarget audience: Doctors\, nurses\, aged care workers \nClick here for more information and to register
