GP aged care connect

Murray PHN launched its new GP Aged Care Connect service in September 2024, to help connect permanent residents of residential aged care homes (RACH) with a GP and GP practice where required.

As part of this service, Murray PHN works with local RACH providers and general practices, to assist with connecting RACH residents with a preferred GP and practice in MyMedicare*, if they do not currently have one. 

Key benefits

  • regular visits, care planning, and health assessment services, leading to improved continuity of care and reduced hospitalisations
  • providers will receive payments for services provided to older people living in aged care
  • practices will receive funding to manage quality care in aged care settings.

New aged care incentive and MyMedicare

The service is part of the wider General Practice in Aged Care Incentive program, that was developed in response to the Royal Commission into Safety and Quality in Aged Care and is part of the Strengthening Medicare Reforms.

Under the new incentive, general practices and GPs will receive funding to support the delivery of more proactive, planned, and continuous care. While not directly funded by the incentive, residential aged care providers have an important and shared purpose in helping people who live in their homes to access quality primary care and other health services.

The MyMedicare General Practice in Aged Care Incentive has been developed in response to the Royal Commission into Aged Care Quality and Safety and the Strengthening Medicare Taskforce and aims to deliver quality care over time.

The incentive started on 1 August 2024 and can only be accessed through *MyMedicare, where both the general practice and residential aged care home resident are registered.
