Residential in-reach (RiR) services are staffed by hospital nurses and doctors. They are regionally available to provide care and support to people living in residential aged care homes to help reduce avoidable presentations to local emergency departments. They are not intended to replace the care that should be received from the RACH or the residents usual GP.

In some circumstances it may be necessary for a resident to go to hospital. RiR can assess and discuss the most appropriate place for the resident to receive care, and advocate for a transfer to the appropriate level of hospital care when required.
