Primary Health Networks play a crucial role in supporting health reform across Australia, by driving local innovation to meet specific health needs and supporting consistent delivery of national and co-commissioned programs to keep people well.

Murray PHN is a not-for-profit organisation, reporting to an independent Board. We are one of six PHNs in Victoria and 29 across Australia, working locally with the national goal of making high quality and equitable primary health services available to all Australians, no matter where they live. Our vision is for healthy rural and regional communities, with timely access to the primary healthcare they need. This means that people in our catchment are healthy and well and have access to high-quality primary healthcare that meets their needs and prevents disease. Our focus is on people in our communities with chronic health conditions or mental illnesses, and on introducing programs at a community level that can help reduce avoidable hospital stays.
