Advance Care Planning in Aged Care | Shepparton

This interactive training workshop is designed to empower healthcare professionals working in aged care with the knowledge, skills, and confidence to engage in meaningful advance care planning (ACP) conversations with older people.

It is ideal for aged and community care workers, nurses, care coordinators, and other health professionals involved in supporting older people in residential or community care settings. Participants representing organisations that provide CHSP, Home Care Packages and residential aged care services within the region are encouraged to attend.
