Meet Hilda
She’s 79, staunchly independent, and determined to stay in her own home. But osteoporosis, bipolar disorder, and possible cognitive decline are making life more complex and more isolating.
At this webinar, a multidisciplinary panel including a general practitioner, psychiatrist, nurse and lived experience advocate will unpack Hilda’s hypothetical story and explore how collaborative care can support older people to live well in the community, even as needs change. Join us to meet Hilda and see multidisciplinary care in action.
