Chronic Disease Care Planning | Online

FreeThe Chronic Disease Care Planning Course (CHCCCS001) is a nationally accredited course that aims to provide students with the skills and knowledge required to manage the client with chronic disease by including care planning, identifying risk factors, identifying and supporting behaviour change, engaging the client, applying health interventions and developing effective care plans.
Aboriginal and Torres Strait Islander health practitioners and workers, allied health professionals, enrolled and registered nurses, GPs and practice managers can attend this course*, to develop an understanding of the key principles of nutrition and associated chronic conditions and create an overall understanding of how a healthy diet can support patient care.
The program is supported by funding from the Australian Government Department of Health under the Chronic Disease Support Project.
*Students must complete the Foundations of Chronic Disease online module to book into this program. The online module is also provided at no cost.
