In response to recommendations made from the Royal Commission into Aged Care Quality and Safety and to improve the health of older Australians at risk of poor health, Murray PHN has received funding to support residential aged care homes with training and equipment, focussing on enhancing virtual and after-hours access to care.
My Health Record is secure and convenient way for residents and their health and care teams to share immunisation records, details about allergies, medicines, test results, and shared health summaries. Having key health information available online and centrally located saves time by chasing information and reduces duplication and risk.
An advance care plan is a digital document that can be uploaded to a person’s My Health Record which provides clear instructions on the person’s wishes for end-of-life care.
Telehealth is not a solution for every health condition but can provide additional reliability and increased access to appointments, particularly in rural areas and in the after hours period, without the need to transport patients and cause them extra distress.
